Schedule a Counseling Appointment

To schedule an appointment with CFS, please call us at 828-262-4951.

If this will be your first visit, we will require two forms from you: an Informed Consent form and a Client Information form. You may download these forms from the Forms section below, print them, and fill them out prior to your arrival. Alternatively, you may arrive 10-15 minutes early to fill them out in our office.

Please note that CFS staff does not use email to schedule appointments or to communicate confidential information.

Rescheduling or Canceling an Appointment

If you need to reschedule or cancel an appointment, please call us at 828-262-4951 with at least 24 hours' notice, if possible.

Confidentiality

All CFS services are confidential within limits imposed by state and federal laws. In most cases, state and federal laws prohibit CFS from revealing any information about you without your consent. Your CFS counselor will discuss legally required exceptions to confidentiality with you at your first meeting, or you may download the Informed Consent form and bring it with you to your first meeting to discuss any questions you might have. Please feel free to bring up the topic of confidentiality with your counselor at any time.

Our counseling staff is highly qualified to serve your needs and guide you when making assessments or referrals. Each staff member is licensed in the fields of counseling, family therapy, or psychology, and they have all gained years of experience working in a wide variety of clinical settings.

Cost

CFS services are free. If a counselor refers you to services elsewhere, charges for those services are the employee's responsibility. You may use health insurance to pay for those services when appropriate, check with your insurance provider for details on coverage.

Appointments During Work Hours

You may be released from work hours if it is not critical to your job, pending your supervisor or manager's agreement. If necessary, you may also utilize employee leave time for the purpose of attending sessions.
